You have to apply to secretary for the commissioner exams, trivandrum in the prescribed format with necessary fee (200 rs pay in tressury), along with that 

1. an affidavit from the head master where you studied.

2. An affidavit from Magistrate stating the SSLC book lost irrecoverably.

3. paper cutting for publishing advertisement statting irrecoverably lost the SSLC Book.

4. If lost the SSLC book aborad, instead of magistrate affidavit you may collect the attested affidavit from embassy.

all these ahve to submit to the secretary to Commissioner of exams.

You get the form in Exam bhavan at tvm or can download from the site keralapareekshabhavan.in/forms/sslc.pdf
