Doctors with conscience speak out
Rema Nagarajan,TNN | Feb 22, 2015, 06.24 AM IST
NEW DELHI: In order to benefit the hospital and meet its commercial needs, one has to do things like keeping patients in the hospital longer than necessary, and doing unnecessary investigations and procedures (including angioplasty) since there was pressure from the management of the hospital.
My conscience began pricking and I left the hospital- Dr Gautam Mistry, Kolkata, cardiologist who left a corporate hospital after seven years.
A reference for angioplasty can earn a doctor Rs 30,000-40,000 - Dr Rajendra Malose, general practitioner, Nashik
Recently, a young doctor who joined our department told me, "Sir, every month there is a meeting with the CEO. He asks me questions because instead of having a 40% conversion rate for OPD-operative as per the target, my conversion rate is just 10-15%. (Conversion rate means out of all patients seen by the doctor, how many are advised to undergo surgery or procedures. Rational doctors try to keep this rate low, but profit-driven hospitals try to maximise number of surgeries and procedures, even if they are unnecessary). He tells me that such low conversion rate will not do, and that unless I increase it, I will have to leave the hospital." This young doctor will certainly surrender one day. To survive professionally, he will start doing 20-25% of additional procedures that are not required by medical logic. What choice does he have?"... And each corporate hospital has such targets! There is no getting out of it. - Super specialist from a metro
Pharma companies are giving foreign tours and junkets to doctors. It happens under the pretext of medical study. Unfortunately, some doctors eagerly wait for the pharma company invitation for foreign tours- Dr HV Sardesai, physician Pune.
Corporate hospitals only want doctors who can help them earn more money. As a result doctors who practise ethically cannot last there. I know of a hospital where if a patient is charged Rs 1.5 lakh, the doctor gets a mere Rs 15,000. 90% of the income goes to the corporate coffers. Corporate hospitals can advertise while individual doctors are not allowed- Dr Sanjay Gupte, gynaecologist, Pune, ex-national president of the Federation of Obstetric and Gynaecological Societies of India (FOGSI)
These are just a few of the shocking revelations by 78 doctors from small towns to every one of the megacities who are critical of the growing commercialisation of medical care. The doctors range from general practitioners to super specialists in corporate hospitals. These interviews that expose the corruption in private healthcare have been put together by SATHI (Support for Advocacy and Training to Health Initiatives), an NGO, to highlight the lack of regulation of the sector.
A report based on these interviews titled, "Voices of Conscience from the Medical Profession: Revealing testimonies by rational doctors about the reality of private medical practice in India" has been put together by Dr Arun Gadre, a doctor and writer with 20 years' experience of working as a gynaecologist in rural Maharashtra, and Abhay Shukla, convenor of SATHI.
The report will be released at the All India Institute of Medical Sciences (AIIMS) on February 26, in a function to be attended by AIIMS director Dr MC Mishra, senior gastrointestinal surgeon Dr Samiran Nundy of Sir Ganga Ram Hospital, and several of the doctors from across the country who have spoken out in the report.
The report is an English translation of the recently published Marathi report 'Kaifiyat - pramanik doctoranchi', which is being widely read in Maharashtra and is already into its second edition. An enlarged version of this report is soon to be published as a book.
"These 'whistleblower' doctors have exposed, perhaps for the first time on such a scale and in so many dimensions, the realities of the private medical sector today such as frequent irrational procedures and surgeries, the distorting influence of corporate and multi-specialty hospitals on ethics of the medical profession, and the growing grip of pharmaceutical companies on private medical practice. With testimonies by rational doctors from across India, this report can be an eye-opener for ordinary citizens as well as doctors, and could strengthen social support for much-needed moves to effectively regulate the private medical sector in India," explained Shukla.
According to him, the government is trying to dilute the Clinical Establishments Bill on the grounds that outdated laws have to be changed. "The bill has not even become law and no proper implementation of any law to regulate the private medical sector has been undertaken and even before that you are saying it is outdated. There is a strong lobby of the corporate health sector and the Indian Medical Association, the biggest lobby of doctors in India, that are trying to completely eliminate any kind of regulation. It is total jungle raj now. This is the larger policy environment in which we are releasing the report," said Shukla.
Public health activists have stressed the need to urgently step up regulation of the private health sector rather than dilute whatever little regulation exists. "Doctors have their lobbying groups like the IMA, which will speak of their interests. Society needs to speak up and lobby for the interests of the patients," said Shukla.

[www.keralites.net] Will Eating Tomatoes Lead To Kidney Stones?

Will Eating Tomatoes Lead To Kidney Stones?

Indian cooking is incomplete without all the spices and ingredients like garlic, chili, onion and tomatoes. You can hardly avoid these when you relish any typical Indian dish. Now comes the million dollar question- will eating tomatoes lead to forming of kidney stones in your body?
To know this, let us find out how kidney stones are formed-
Interestingly, you may not know that you have a kidney stone, till it starts moving within your kidney or gets into your ureter. That's when you start experiencing symptoms like severe pain in and around your ribs and lower abdominal area, nausea, increased urination, smelly urine and painful urination often with traces of blood in it.
Kidney stones often occur due a number of factors. Simply speaking, kidney stones form when there are substances in your urine which help form crystals. Uric acid, calcium and oxalate are examples of such substances. Also at the same time, if your urine does not have compounds that stop the crystals from sticking together, then stones occur.
Food that you eat plays a major role in forming kidney stones. Avoiding such foods will definitely lower the risk of stone formation.
So, what do tomatoes contain that may result in forming a kidney stone?
It is a good idea to clearly understand the link between tomatoes and kidney stones to avoid getting flustered over this issue. Normally eating tomatoes or tomato seeds does not result in kidney stones.
However, eating tomatoes may not be a good idea after all for those people who already have a tendency of developing kidney stones or have kidney related problems. There is a reason for this.
Tomato seeds have a lots of oxalate content. When this oxalate comes in contact with calcium in the body, it forms a compound known as calcium oxalate. This compound leads to forming of crystals in the kidneys eventually leading to formation of kidney stones.
So, if you already have kidney stones in your body or have the tendency to develop them, then you should keep away from consuming tomatoes in any form like in salads, dishes, sauces, puree etc. This will lower your risk of getting kidney stones.
One more thing to keep in mind is that if you already have kidney stones then avoid eating green tomatoes seeds. They are harmful for your kidneys. This is because these seeds are very difficult to digest and may lead to formation of more kidney stones if you eat them too frequently.
Tomatoes are a minefield of fibre, minerals, vitamins, lycopene and other antioxidants that boost your health tremendously. You should avoid its consumption in any form only if you have kidney stones.

[www.keralites.net] AN IMPORTANT VITAMIN THAT YOUR DOCTOR NEVER TALKS ABOUT

An Important Vitamin That Your Doctor Never Talks Of
Many times as a kid, you might have fallen down and got your knees scraped. The blood would come out and slowly, over the days, the wound would heal. Basically, it was your system's own natural way of healing your scratches by congealing our blood from liquid to gel form, which, is also known as the process of "coagulation".
Have you heard about friends or family, that their blood doesn't clot easily? Or, that even a slight scratch might remain "fresh" for a longer time. It is due to the delaying in blood clotting. Blood clots, a very simple biological procedure, which you studied in middle school biology, are responsible for healing the wounds. They form due to the presence of Vitamin K, in the absence of which you may bleed to death in serious injuries.
What is Vitamin K?
Vitamin K includes phylloquinone (Vitamin K1) and menaquinone (Vitamin K2). In a layman's words, it is basically a vitamin that is found in abundance in green leaves and is essential for the blood clotting process. As kids you may not have been too fond of consuming green veggies, no matter how much your mom coaxed or threatened you to have those. Sigh! But then, they say, that health is wealth.
Why you should take Vitamin K? Can the deficiency be harmful?
It is a very good question, given the fact, how we all go "ewww" at the very mention of consuming leafy veggies (unless from "Subway"). smile emoticon Vitamin K helps in blood clot, preventing excess bleeding. This is the first reason why we should include Vitamin K in our diet. Did you know that newborns are injected with Vitamin K? Ever wonder why? The reason is that newborns are more prone to Vitamin K deficiency than adults.
Deficiency of Vitamin K can lead to uncontrolled bleeding, that much is obvious now. However, there are certain other cases that pose a higher threat of Vitamin K deficiency.
Have a disease that affects absorption in the digestive tract, such as Crohn's disease or active celiac disease
Take drugs that interfere with Vitamin K absorption
Being malnourished
High alcohol consumption
In these aforementioned cases, your doctor might suggest Vitamin K supplements.
How much Vitamin K should you get?
This is a very important thing to keep in mind. We have always been told, no matter how nutritious something might be, in excess, it might have effects we may not like. Not to forget that the excess can completely neutralize the positives that we gained from it earlier. Same is the case with Vitamin K. So, I am presenting to you, the quantity per day of Vitamin K, that different people should consume-
Group (years) Adequate Intake
Children 0-6 months 2.0 micrograms/day
Children 7-12 months 2.5 micrograms/day
Children 1-3 30 micrograms/day
Children 4-8 55 micrograms/day
Children 9-13 60 micrograms/day
Girls 14-18 75 micrograms/day
Women 19 and up 90 micrograms/day
Women, pregnant or breastfeeding (19-50)Women, pregnantor breastfeeding(less than 19) 90 micrograms/day75 micrograms/day
Boys 14-18 75 micrograms/day
Men 19 and up 120 micrograms/day
What are the foods that can provide a healthy dose of Vitamin K?
Now comes the main part. We have read the benefits of Vitamin K, and what harms the deficiency might cause. Now let us have a quick peek at the foods that are rich in it.
(The ones in bold are the main and best sources, the rest are other alternatives).
Herbs : Dried Basil, Dried Sage & Dried Thyme, Fresh Parsley, Dried Coriander (Cilantro) & Dried Parsley, Dried Marjoram, Fresh Basil and Fresh Chives.
Green, Leafy Vegetables: Kale (cooked), Frozen Kale, Frozen Spinach, Mustard Greens, Spinach, Collards, Beet Greens, Swiss Chard, Turnip Greens, Dandelion Greens and Broccoli Raab.
Salad Vegetable: Spring Onions / Scallions, Garden Cress, Endive, Radicchio, Chicory Greens, Watercress, Cos, Lettuce, Green Lettuce, Red Lettuce, Celery, Arugula, Iceberg Lettuce & Cucumber.
Brassica Vegetables: Brussels Sprouts (cooked), Broccoli, Cabbage, Frozen Broccoli, Chinese Broccoli, Red Cabbage, Pak Choi, Savoy Cabbage, raw and Cauliflower.
Hot Spices: Chili Powder, Curry Powder & Paprika and Cayenne Pepper.
Other Vegetable: Asparagus (cooked), Frozen Asparagus, Leeks, cooked, Okra, cooked and Fennel, raw.
Pickles: Cucumber (sweet), Dill Pickle, Sour Pickle and Sweet Pickle Relish per tablespoon.
Soybeans (cooked), Raw Soybeans and Roasted Soybeans.
Vegetable Oils: Olive Oil, Soybean, Canola (Rapeseed) and Sesame Oil.
Dried Fruit – Prunes, Blueberries, Pears, Peaches, Figs and Currants.
Phew! Quite a long list, isn't it? But, on the good part, this list gives a long list of alternatives of our own choice that we can consume for our healthy dosage of Vitamin K.
We should always take the good in things, with a pinch of salt. Though the side effects in Vitamin K dosages are rare, but again, no matter how small, these cannot and should not be ignored. I am sure, you would agree too, after having a look at these.
Many drugs can interfere with the effects of Vitamin K. They include antacids, blood thinners, antibiotics, aspirin, and drugs for cancer, seizures, high cholesterol and other conditions. People using medications for heart problems, clotting disorders, or other conditions may need to watch their diets closely to control the amount of Vitamin K they take in. They should not use Vitamin K supplements unless advised to do so by their health care provider.
Before I conclude, all I can say is that though Vitamin K is very essential and beneficial to us. It should only be taken as a supplement, if your doctor prescribes it, else not, if you want to escape the side effects. Be assured, your normal daily diet contains sufficient dosage of Vitamin K.

[www.keralites.net] Baba Ramdev's 'new fan' Lalu Yadav says people are jealous of Yoga guru

Baba Ramdev's 'new fan' Lalu Yadav says people are jealous of Yoga guru

Delhi: Though Baba Ramdev has attacked veteran politician Lalu Prasad Yadav several times in past, the RJD chief appears to have become an ardent fan of the renowned yoga guru.

The former chief minister of Bihar, who met Baba Ramdev in the national capital on Wednesday, said, ''People are jealous of Baba Ramdev because he is highly successful.''

Praising the yoga guru, Lalu said, ''His products are in demand and he is doing good for the country."

The veteran Bihar leader, in a joint press conference with yoga guru, said that whatever Ramdev earns, he spends it on charity.

Defending Baba Ramdev over various allegations of accumulating huge wealth through illegal means, Lalu said, ''he has earned everything through sheer hard work.''

It was a conspiracy by his rivals to defame him, his products, the RJD chief said.

When quizzed by reporters whether he will promote Baba Ramdev's products, Lalu jokingly said, ''I am a permanent brand ambassador of his products.''

On his turn, the yoga guru said he had come to invite Lalu Yadav to attend International Yoga Day celebrations.

He also gifted Lalu Yadav several products of his Patanjali Ayurved. The yoga guru also tweeted a picture of his meeting with the RJD chief.
