The speed and quality of wound healing aspects depend on the characteristics of the lesion and its location

Cut with a knife or fall in the park are easily accidents can cause   wound   which it is cured in a few days. However, in some people, the healing process lengthens and injuries do not close until after a few weeks. Why? What aspects influence a good or bad scarring? This article addresses these and other   issues related to wound healing. Also described what they are  first aid to a wound.

The process of wound healing

At the same moment an injury occurs, the body initiates an imbricated process with the aim of restoring the tissues. The scientific literature this process divided into three phases.

In the   inflammatory phase, a certain cells (macrophages) and remove dirt and bacteria migration and proliferation of other acting in the next stage originates.

In the   proliferative phase, which will begin two to five days after the injury occurred, new blood vessels , which bring oxygen and nutrients necessary to the area- are created, increased collagen fibers , which form the structural and constitutes the need to replace tissue that has been damaged. An acute injury should be returned before 10 days.

The last phase, called for   maturation and remodeling, is the longest and may last even more than a year. In this period, the collagen fiber used to repair the wound is replaced by other more resistant and blood vessels that were created for recovery are also destroyed. Here, the color of the wound begins to lose flushed tone.

Factors influencing healing

Before cure any injury, it is important to wash their hands thoroughly and thorough drying

The speed and quality of healing of a wound rely on particular aspects, such as genetic factors and characteristics of the lesion (tissue loss, characteristics of the edges of the lesion, presence of devitalized tissue and / or edema, degree amount of dirt or germs). Thus, a deep wound with loss of tissue and the edges has not been possible to approximate have a slower than a clean wound edges that may have been sutured without further evolution. But it also depends on their location, as scarring is more complicated if you are on joints in wet areas or those suffering from a continuous friction, as the soles of the feet.

However, there are other factors that can determine the evolution of this process, as detailed Dr.   J. Javier Soldevilla   in the 'Practical Guide in the care of skin ulcers':

  • Age. The skin is a living organ in continuous regeneration, but, from 25-30 years, begins to shrink its capacity for renewal: morphological changes occur, elastic fibers are lost and reduces their blood supply. In addition, the aging process involves hormonal changes, such as those that occur in the   menopause , the immune system , which protects against pathogen - be depressed and more danger of infection runs. In the same way, as years are met, there is more chance of developing a chronic disease that has direct involvement in healing.
  • The state of nutrition and hydration. Both obesity and malnutrition are factors that hinder the healing process due to lack of vitamins (A and C), anemia, hypoproteinemia (deficit protein intake responsible for providing the substrate for the formation of collagen , important for its structural function), the deficit of minerals such as zinc (involved in tissue repair) and the lack of hydration (which causes the scar tissue is weaker). In addition, dehydrated skin is more vulnerable to external aggressions.
  • Comorbidities. Cardiovascular or respiratory diseases, allergies, diabetes or infection alters the healing process is a result of the condition itself in the body (poor blood circulation, reduced collagen deposition, very depressed immune system, lack of oxygen in tissues) or effect of the drugs used in their treatment (corticosteroids, cytotoxic, immunosuppressive drugs).
  • The consumption of snuff and alcohol. The usual making these substances causes changes in blood vessels and limits the flow of blood to all layers of the skin, changes in blood clotting or reduction in cells directly involved in healing (decreased platelets, collagen destruction and decreased hemoglobin, among others).

First aid to a wounded

Although never be manipulated with fingers blood nor a wound bed, it is important, before cure any injury, perform   hand washing   thoroughly with soap and water, without forgetting a thorough drying.

The next step is   clean the wound. Any wound is contaminated by microorganisms that exist in the skin, but proper cleaning will help, along with the immune system of each, they do not proliferate and an infection develops. The purpose of cleaning is to remove detritus, as traces of soil or dead cells, which can hinder healing and encourage infection. To clean the wound -preferible simply apply a saline solution at a temperature of 32-35 ºC- or, failing this, potable water.

If the wound is very dirty, you can soap and water for cleaning used, but be very careful to rinse and leave no rest. You can also use a   antiseptic solution   that is effective for a large number of germs, such as chlorhexidine, and according to the therapeutic indications attached in the prospectus. Other antiseptic solutions used (povidone-iodine, alcohol or hydrogen peroxide) may cause adverse effects on the skin such as irritation or allergic reactions important, among others, as well as slowing the healing process.

If convenient, you can   cover the lesion   with a non adherent dressing (hydrocolloid or polyurethane foam). Cotton is not recommended, as it is easy to adhere to the tissues and cause pain and bleeding when wanting to remove it; and if it is not removed completely, their remains can cause an infection.

If the wound is deep, it has embedded objects or is very dirty and you can not clean in the usual way, it presents a bleeding difficult to control or is caused by the bite of an animal or materials that could be contaminated by fungi or bacteria, must go to a health center for the values a healthcare professional.
